Supporting Recovery, Function, and Independence
Rehabilitative and therapeutic support services are being developed to help individuals work toward improved function, mobility, and independence at home. Services will be provided in accordance with applicable practitioner orders, individualized plans of care, and professional scope-of-practice requirements.
Support may include reinforcing prescribed therapeutic activities, promoting safe mobility, providing appropriate education, and communicating changes or concerns to the appropriate healthcare professionals. Care will be tailored to the individual’s needs and goals as outlined in the plan of care.
