Ongoing physician-directed assessments are being developed to support appropriate monitoring of an individual’s health needs and changes in condition. Assessments will be conducted in accordance with practitioner orders, the individualized plan of care, and applicable professional scope-of-practice requirements.
Our approach focuses on observing relevant changes, documenting findings, and communicating concerns to the appropriate healthcare professionals. This supports coordinated care and helps ensure that updates to the plan of care are made by the appropriate practitioner when needed.
